Expert Guidance. Unwavering Advocacy. A Trusted Partner Through Every Care Transition.

When a loved one's health changes, you shouldn't have to navigate the system alone. Beacon Pathways combines the expertise of clinical social work and nursing to advocate for you and your family every step of the way.

Navigating a hospital discharge, a move into a new care setting, or growing concerns about a loved one's care can feel overwhelming, confusing, and isolating. Beacon Pathways is here to change that. Our Care Transitions and Coordination service brings together the clinical insight of a Licensed Clinical Social Worker and the medical expertise of a Registered Nurse to give you a true partner in navigating complex healthcare situations.

With over a decade of combined experience in care coordination, discharge planning, and advocacy across hospitals, agencies, and skilled nursing facilities, our team knows this system inside and out, and we are not afraid to use that knowledge to fight for you.
