Our Care Transitions and Coordination service brings together the clinical insight of a Licensed Clinical Social Worker and the medical expertise of a Registered Nurse to give you a true partner in navigating complex healthcare situations.
With over a decade of combined experience in care coordination, discharge planning, and advocacy across hospitals, agencies, and skilled nursing facilities, our team knows this system inside and out, and we are not afraid to use that knowledge to fight for you.
